This survey asks whether you have recently received a flu shot and about your experiences in deciding whether to be vaccinated against the flu and other diseases.
This survey is being conducted by health care researchers at the RAND Corporation, an independent, non-profit research organization. RAND is conducting this survey on behalf of GlaxoSmithKline, a company that makes vaccines to help prevent the flu and other infectious diseases. Your answers will help GlaxoSmithKline and other companies like it, federal, state, and local public health officials, and health care providers understand who is being vaccinated and better communicate to consumers the benefits and risks of vaccines.
It is similar to the other surveys you have already completed. Your response to this survey, or any individual question on the survey, is completely voluntary.
